If you have been matched to an Equinox Doctor, please contact the clinic to set up a “meet and greet” appointment to confirm the match, and complete the New Patient Intake Form below.

In preparation for your first appointment, we request that you complete this form to the best of your ability. It can also be done on paper at the clinic or after you first meet your doctor and confirm your match you prefer.

If you complete a form without having been matched to an Equinox Doctor the information will be discarded and you will not be contacted.
